Preparing For Your First Appointment
We treat our patients with the dedication, care, and expertise they deserve. We are currently accepting new patients and looking forward to talking with you!
If you are a new patient, you will be asked to fill out the New Patient Packet. For your convenience please download, complete, and bring the completed forms with you to your appointment. You will also be asked to fill out your medical history in the EMA patient portal prior to your appointment.
Please arrive 15-20 minutes early to your appointment if you need to complete the new patient packet or EMA patient portal at our office.

What to bring to your appointment.
At the Time of Your Appointment Please Bring:
- Photo ID
- Insurance Cards
- All Diagnostic Tests – i.e., MRI, X-rays, CT scan
- A list of all current medications
- Medical records including surgery notes if applicable
- Co-payments

Accepted Insurances
Sound Hand & Orthopedics contracts with the following insurance companies and forms of payment:
- Medicare – Participating Providers
- PPO – Blue Cross, Blue Shield, Cigna, Sutter Select, United Healthcare, Others
- Commercial Insurances
- HMO (Brown and Toland, Alta Bates Medical Group, Sutter East Bay Medical Foundation)
- Credit and Debit Cards (Visa, Mastercard), Cash, Check
To determine if we participate in your insurance plan, please call your insurance company or look in your participating network guide prior to your visit.
We collect all co-pays and outstanding balances at the time of visit.

Appointments
Appointments will last about an hour. You should come to your appointment with all of the required documentation. If you have specific questions about what to expect during your appointment, please call our office at (206) 257-3350.
